Bespoke Person-Centred Care

We provide high quality, bespoke person-cantered care, and support for individuals of  aged 13-17 and 18-65 years old. This service is designed to support individuals with a learning disability and/or autism with highly complex and severe behaviour which challenges services and associated risks. 

We place the needs of our residents at the centre of all we do, promoting choice and independence in an environment that respects privacy, rights, and dignity.  They must be an equal partner in the planning of their care and that their opinions are important and will be respected. Being truly person-centred means that we think about the effect of the care plan on the person as a whole.
Personalised care planning aims to provide support from care professionals that is tailored to the needs of each individual. It involves a conversation, or series of conversations, between the individual and the care team when they jointly agree on goals and actions for managing the service users` problems. Individuals goals in managing their condition may differ from those of professionals, but both are important. Instead of giving instructions and advice, this approach encourages service users to clarify their own goals, working together with the therapist to identify the help they need to achieve these.
